Meeting Families Where They Are: Developing and Implementing a Tiered Care Coordination System for Youth Behavioral Health Needs
September 24, 2024 | NWI
Wraparound addresses the needs of young people with complex behavioral health challenges who are involved in multiple systems and at risk of out-of-home placement. However, many families and young people could benefit from care coordination without requiring the intensity of Wraparound. FOCUS is an evidence-informed care coordination model designed for youth and families whose needs do not necessitate high-intensity care but still require support to prevent future system involvement. By integrating FOCUS alongside Wraparound, a tiered care coordination model can be implemented, offering more tailored levels of care that better match families’ varying needs. This webinar is intended for educators, mental health professionals, clinicians, care coordinators, and system leaders interested in learning how a tiered care coordination model could benefit the families they serve. The session will also provide an overview of key considerations for implementing a tiered care coordination model.
Understanding Tiered Care Coordination: Why It Matters
Tiered care coordination models offer a structured, responsive framework to support varying levels of behavioral health needs among youth. These models ensure that youth receive care coordination that aligns with their specific level of need, maximizing both effectiveness and efficiency in care delivery. This webinar will highlight two care coordination models that have been used to develop a tiered care coordination approach:
- Wraparound: A comprehensive, family-driven, and youth-centered approach assembles a team of professionals and natural supports to address complex needs. Wraparound coordinates care across multiple domains—mental health, education, family support, and more—ensuring that services are integrated and holistic, rather than fragmented.
- FOCUS: A newly developed care coordination model designed to support youth and families with less complex needs, who are system involved, at risk of deeper or multi-system involvement, and whose challenges exceed the resources of a single organization. It aims to decreased system involvement while building connections and supports for families through community-based resources.
