HHS Federal Partners Integrated Care Meeting (Day 1)

"State of the Art: Research, Models, Promising Practices and Sustaining Integrated Care" this two day virtual meeting is open to the public and any interested organizations. It will highlight models of integrated care, key findings from the research community, examples of diverse grantee practices regarding service delivery, presentations by U.S. Department of Health and Human Services agencies, and a wide range of resources to support efforts to build integrated systems of care.
